Operatieverslag Dynamic Hip Screw (DHS - Synthes)
Indicatie: (Pertochantere femurfractuur / Mediale collumfractuur) (rechts/links) bij ..-jarige (man/vrouw)Verslag: Time out procedure. Antibiotische profylaxe Cefazoline 1000 mg i.v. Patient in rugligging op tractietafel onder (spinaal/algeheel) anaesthesie. Het contralaterale been wordt abduceerd. Met röntgendoorlichting met behulp van een K-draad wordt de positie en richting van het collum en het femur worden bepaald. Onder röntgendoorlichting wordt de fractuur onbloedig gereponeerd. Huiddessinfectie middels Chloorhexidine en steriel afdekken. Lengteincisie 15cm caudaal van het trochanter major. Klieven fascia lata in vezelrichting, lengte-incisie fascie vastus lateralis, van distaal naar proximaal werkend de vastus lateralis vrij-prepareren 1 cm craniaal van de fascia intermuscularis en omklappen naar vertraal. Hohmannse haak plaatsen over de femurschacht. Boren van een K-draad (2,5 mm) door de laterale cortex in het femur onder röntgendoorlichting tot net subchondraal van het caput femoris met behulp van een 135o-richtapparaat. De draad ligt in AP-richting caudaal in het collum en centraal in het caput. In laterale richting ligt de draad dorso-centraal in het collum en caput femoris. Meten van de lengte van de in te brengen schroef, 105mm. De drietaps boor instellen op juiste lengte en over de K-draad boren tot in de kop waarna de collum schroef wordt ingebracht. Er wordt (wel/niet) getapt. Inbrengen van de 105mm schroef tot net subchondraal waarbij de winding voorbij de fractuur ligt en er zorg voor wordt gedragen dat het handvat aan het eind van de procedure evenwijdig loopt met de femurschacht, zodat het plaatdeel ook evenwijdig kan worden aangebracht. Plaatsen van een 2-gats 135o-plaat zonder compressieschroefje. Met drevel plaat tegen de laterale cortex tikken. Inbrengen van neutrale corticalis schroeven door beide cortices van beide 42mm. Opheffen tractie. Onder röntgendoorlichting wordt een goede stand van de fractuur en positie van de DHS vastgelegd. Controle hemostase. Sluiten fascia lata met Vicryl 2.0. De huid word gesloten met Ethilon 3.0. Sign out procedure. 20cc bloedverlies.
