Een fistel is een abnormale connectie tussen twee geëpithelialiseerde oppervlakten (cq. organen).Enterische fistels zijn verbindingen tussen het gastro-intestinale systeem en andere organen

Klinische presentatie
- Typisch beloop: 4
- - Wondinfectie na 5-10 dagen post-operatief
- - Gevolgd door incisie en drainage
- - Vervolgens darminhoud uit de wond
- Bij intra-abdominale infectie/abcessen:
- - Abdominale pijn
- - Misselijkheid en/of braken
- - Obstipatie
- - Tekenen van darmobstructie
- - Systemische tekenen zoals koorts, koude rillingen of eindorgaanschade
- - Multi-orgaanfalen
- Specifieke klachten, afhankelijk van segment:
- - Diarree (fistel tussen proximale dunne darm en distale colon, of galwegen en dunne darm)
- - Pneumaturie en recidiverende UWIs (colovesicale fistel)
- - Ontlasting via vagina (colovaginale fistel)
- - Dunne darm obstructie (inflammatoire massa, abces, tumor, hernia, adhesies, galsteenileus)
- - Gastro-intesinale bloeding (aorta-enterale fistel)
Diagnostiek
- Aantonen van fistel bij twijfel
- - Verdenking enterocutane fistel - Methyleenblauw (Methylthionine FNA) per os of via NMS
- - Verdenking enteroviscale fistel - Maanzaadtest
- CT-abdomen met oraal en intraveneus contrast (gouden standaard):
- - Anatomie van fistel
- - Intra-abdominale abcessen
- - Distale darmobstructie
- - Pneumobilie
- Gastro-intestinale contrast studie
- - Small bowel follow through (SBFT)
- - Contrast klysma
- Fistulogram
- - Intestinale continuitiet
- - Evaluatie distale obstructie
