4-5-6 protocol

• Hb <4 mmol/L (of Ht <0,18)
• - Acuut bloedverlies op één locus bij ASA I, <60 jr, normovolemisch
• - Chronisch-anemisch asymptomatische patiënten
•  
• Hb <5 mmol/L (of Ht <0,23)
• - Acuut bloedverlies op >1 locus bij ASA I van >60 jaar, normovolemisch
• - Acuut bloedverlies op meerdere loci (polytraumapatiënten) bij ASA 1, < 60 jaar, normovolemisch
• - Pre-operatief bij patiënt met een te verwachten bloedverlies > 500ml
• - Koorts
• - Post-operatieve fase (ongecompliceerd na openhartchirurgie
• - ASA II of ASA III, ongecompliceerd
•  
• Hb <6 mmol (of Ht<0,28)
• - ASA IV
• - Patiënt die niet in staat is het hartminuutvolume te verhogden ter compensatie van hemodilutie
• - Septische of toxische patiënt
• - Patiënt met ernstige longziekte
• - Patiënt met symptomatische cerebrovasculaire ziekte
• - Patiënt met acuut myocardinfarct of instabiele angina pectoris
