Oorzaak
  • - Val met de voet in hyperdorsaalflexie in combinatie met abductie in de midvoet
  • - Hoog energetisch trauma (direct inwerkend geweld)
  • - Synoniem: Nutcracker fracture



Lichamelijk onderzoek
  • - Zwelling en hematoom
  • - Pijn aan laterale voetrand



Diagnostiek
  • - X-voet AP, lateraal en 3/4-opname
  • - CT-scan

Conservatieve behandeling
Indicaties:
  • - Niet gedisloceerde fracturen
  • - Ernstig comminutieve fractuuren zonder reconstructie mogelijkheden


(Na-)behandeling:
  • - 6 weken onbelast onderbeengips


Follow-up:
Poliklinische follow‐up

- Gipswissel naar circulair kunstof onderbeensgips (COBG) 		‐ X‐voet

Operatieve behandeling
Indicaties:
  • - Intra-articulaire fracturen met >2mm dislocatie (die te fixeren zijn)


(Na-)behandeling:
  • - Bloedige repositie en schroeffixatie eventueel i.c.m. overbruggende externe fixateur van calcaneus naar MT4 of MT5
  • - 12 weken onbelast, eventueel vanaf 6 weken partieel belasten (20kg)


Follow-up:
Poliklinische follow‐up

- Hechtingen verwijderen

  • - Wondinfectie

  • - Uitbreken osteosynthese materiaal
  • - Zenuwletsel (n. peroneus superficialis, n. suralis)
  • - Pijnklachten en/of functieverlies ondanks geslaagde osteosynthese
  • - Nonunion
  • - Diep veneuze trombose
  • - Artrose art. calcaneocuboidea (gewricht van calcaneus met cuboid)
  • - Artrose artt. tarsometatarsales (gewricht van cuboid met MT 4-5)
